Treatment for coronavirus infection

In accordance with the law “On the Fundamentals of Protecting the Health of Citizens in the Russian Federation”, diagnostics and treatment for COVID-19 are fully covered by the compulsory medical insurance policy. Contrary to popular misconception, a medical organization cannot refuse emergency care to a patient, i.e. in a condition that threatens the life of the patient, regardless of the method of hospitalization – in an ambulance or on their own.

According to the instructions of the Ministry of Health, the diagnosis of COVID-19 is made as a result of a whole range of diagnostic measures. The doctor must conduct an examination, listen to the lungs, measure temperature, blood oxygen saturation indicators, and, if the clinical picture is consistent, refer the patient for “coronavirus diagnostics”: donate biomaterial, smears and / or blood, for tests, and, if necessary, undergo an x-ray of the chest organs cells, computed tomography of the lungs, if necessary, ECG and other methods of laboratory and instrumental research.

If a patient is diagnosed with COVID-19, depending on the severity of the condition and the degree of organ damage, the doctor should prescribe a complex drug treatment, consisting of a combination of drugs from various groups: antiviral, antimalarial, antibacterial, antipyretic. Patients should receive all these medicines free of charge only in case of hospitalization. When providing medical care in a polyclinic, the drugs are not financed from the MHI funds, they will have to be bought at their own expense.

What is included in the dispensary

Medical examination is a preventive medical examination. In our country, free medical examination has been operating since 2013. Surely, you were offered to go through it during a visit to the district clinic. Not everyone is eager to agree, some believe that this is done for show …. In fact, the goal is an important one: to identify the patient’s signs of diseases that the person himself is not yet aware of. Including oncology.

At the age of 18 to 39 years, a medical examination can be taken under the CHI policy once every three years. And since forty years – every year. At the first visit, the therapist will collect an anamnesis, that is, find out your complaints, the presence of chronic, hereditary diseases. Measure height, body weight, blood pressure. With the help of tests, total cholesterol in the blood, glucose level will be determined, you will be given a fluorography of the lungs, electrocardiography, intraocular pressure will be measured, and women will be examined by a gynecologist. Plus a set of screenings for cancer. In patients older than forty, the package of analyzes has been significantly expanded, you can find out more about them in your clinic.

High-tech medical care

Many modern treatments for various diseases, such as oncology, fall into the category of so-called high-tech care (HTC). Previously, the entire VMP was paid directly from the federal budget, and it could only be obtained under a quota. Quotas are a complex bureaucratic procedure, therefore, in order to increase the availability of VMP, many of its types, starting in 2014, were transferred to the basic CHI program.

VMP is a specialized medical care, which is provided only for medical reasons, which are determined by the attending physician. The doctor needs to draw up a package of documents and write out to the patient a referral to a medical organization that provides high-tech medical care in the required profile.

Here are just a few of them:

  • stenting of coronary vessels;
  • implantation of a cardioverter-defibrillator;
  • reconstructive-plastic, microsurgical, extended-combined surgical interventions for malignant neoplasms, endoprosthesis replacement of limb joints;
  • the use of genetically engineered biological drugs in the treatment of rheumatoid arthritis;
  • complex surgical treatment of glaucoma.

Assistance in rehabilitation

After complex treatment or long hospitalization, in case of certain diseases and conditions, patients require assistance in recovery – for example, patients with spinal fractures, craniocerebral and other injuries, after heart attacks and strokes. You need to start recovery as early as possible: for example, after a stroke, within the first day after resuscitation.

Therefore, the CHI program includes rehabilitation. It is provided both in multidisciplinary hospitals and polyclinics, outpatient and day hospital conditions, as well as in rehabilitation centers. Rehabilitologists will conduct a comprehensive diagnosis of the patient’s condition, form an individual rehabilitation program and help restore the patient’s lost or limited abilities. Here are some types of help:

  • physiotherapy;
  • physiotherapy;
  • massage and manual therapy;
  • psychotherapy;
  • reflexology.

IVF and other reproductive technologies

About 20% of couples who want to have a baby experience infertility. IVF (in vitro fertilization) procedures, including ICSI (intracytoplasmic injection), have established themselves as the gold standard among fertility treatments, but they are difficult to perform and require special medical skills and equipment, which means that paying for them yourself can be difficult.

Starting in 2016, two IVF or ICSI procedures per year are included in the federal health insurance program. Under compulsory medical insurance, it will be possible to do everything you need for free: ovulation stimulation, follicle puncture, egg fertilization, embryo selection, their cultivation and planting in the uterine cavity. Since 2018, the state even pays for the cryopreservation of embryos.

In order to get IVF under compulsory medical insurance, you need to be observed by a gynecologist in a public or private clinic participating in the implementation of the compulsory medical insurance program, have medical indications for the procedure, undergo the necessary examinations as prescribed by the doctor and collect a package of documents for submission to a special commission.

Vaccination against infectious diseases

The list of diseases for the prevention of which vaccination is carried out is described in a special document – the National Calendar of Preventive Immunizations (NCV). The NCV contains a list of diseases, the optimal age and frequency of vaccine administration. The vaccination schedule for each patient depends on the state of health at the time of the proposed vaccination, age, the presence of chronic diseases and risk groups. All vaccinations included in the calendar are provided under the CHI policy free of charge.

The NPP consists of two parts:

  • Vaccinations against common infections (measles, rubella, mumps, whooping cough, chickenpox, diphtheria, influenza, tuberculosis, hepatitis B, tetanus, polio, Haemophilus influenzae type B).

  • Vaccinations according to epidemiological indications – tick-borne encephalitis, leptospirosis, tularemia, anthrax. They are carried out at certain times of the year and in regions with an epidemiological risk of contracting a particular infection. Both residents of these regions and those who are there temporarily are vaccinated. Vaccination is also carried out for patients at risk for the possibility of infection with especially dangerous infections such as hepatitis A, typhoid fever or cholera.

How to get free medical care

In order to receive any medical assistance included in the compulsory medical insurance program, it is enough to contact the clinic participating in the implementation of the compulsory medical insurance program, having a passport and compulsory medical insurance policy with you.

However, it is important to remember that, unlike the basic CHI program, territorial programs may vary depending on the region. For example, the list of types of VMPs and methods of rehabilitation in different subjects of the federation is different. In order to figure out what exactly you are entitled to under compulsory medical insurance and in what terms they are obliged to provide assistance, you should study the information on the websites of the regional Ministry of Health.

“In addition, any issue related to the provision of medical care under compulsory medical insurance will be resolved by insurance representatives free of charge. They will delve into the specifics of each situation, advise on the possibilities and types of treatment available under compulsory medical insurance, take care of your rights to high-quality free medical care, ”comments Elena Bolshakova, doctor, head of insurance representatives of the AlfaStrakhovanie-OMS Directorate of Medical Expertise.
